What Is Health Home?
Health Home is a free program for qualifying individuals that offers many benefits to them in addition to their established health care and service(s) provider(s). Health Home can also help establish care and/or services if the individual currently is not attached to a health care and/or service provider. All individuals (waivered and non-waivered) qualifying for the DD Health Home program were automatically enrolled by the Division of Developmental Disabilities and assigned to an agency providing this service. Individuals identified as qualifying that were not yet auto-enrolled also can self-enroll or be referred for enrollment by the organizations they are involved with.
